Student-Athlete Forms

To student-athlete and parent/guardian,

Below you will find links to the necessary forms for your son or daughter to begin (or continue) participation as a student-athlete at Tennessee Tech University. Please fill out each form completely. You have two options --
   1) Download the form to your computer, fill it out and email the completed form to the email address below, or
   2) Open the form, fill it out and print it. Then mail it back to the address below.

NOTE - To fill out the form and save the information, you MUST download it to your computer FIRST. If you fill it out without downloading, you cannot save the information or email it.

Please complete each form with signature prior to mailing.

Remember to please fill out each form as accurately and completely as possible. Please sign forms and return all completed information to the Head Athletic Trainer. This will ensure that your file is active when you arrive on campus. Please remember, student-athletes are not allowed to practice until all forms are completed.
